Efficacy of tacrolimus ointment and a new formulation of oral cyclosporine in the treatment of graft-versus-host disease. Adult patients with acute or chronic graft-versus-host disease (GVHD) of the skin, mouth, and gastrointestinal tract were treated with tacrolimus ointment, 0.1% topical (s.c., 2-4 times per day) or oral, 10 mg/kg/day. Those with a good response to oral cyclosporine (CSA) and who were tolerant of CSA were switched to the ointment. Those who failed with oral CSA received a new formulation of oral CSA, 2 mg/kg/day. Thirty-four patients were treated: 18 acute, 11 chronic, and 5 refractory. Sixty-nine percent of patients with acute GVHD and 100% of patients with chronic GVHD achieved a clinical improvement in GVHD. Patients who failed with oral CSA treatment were significantly more likely to develop new GVHD in the second phase of the study. The relapse rate was 46% and, interestingly, the majority of these relapses occurred in the first 12 months. This study demonstrates the efficacy of both tacrolimus ointment and a new formulation of oral CSA in the treatment of acute and chronic GVHD.The protective role of IL-4 and IL-10 on NF-κB activation induced by antigen and Mycobacterium tuberculosis infection in human macrophages. The aims of this study were to investigate the effect of IL-4 and IL-10 on NF-κB activation induced by antigen and M. tuberculosis infection in human macrophages. Isolated human macrophages were stimulated with antigen and M. tuberculosis for 24h or exposed to M. tuberculosis for 1h followed by 24h stimulation with IL-4 or IL-10. In all cases, the cells were then stained for NF-κB and p65. IL-4 reduced NF-κB activation and p65 nuclear translocation induced by antigen or M. tuberculosis infection, but did not modulate NF-κB activation induced by live M. tuberculosis.
